Getting a little leaner would be the first step.
 
Surgery wise
Rhino , infras and some botox
 
fix under eye support with infraortbital implants
lip fillers only a bit tho to help with the philtrum chin ratio
 
Some lashes and some lip liner would b helpful!!!
