you can struggle with food without having a restrictive eating disorder like anorexia though
maybe she had binge eating disorder or bullimia or just a lot of shame around eating

I think a lot of the time people just lie for attention, but it's unfair to say everyone who isn't skinny and isn't starving can't possibly have an eating disorder that deserves concern and care. Also, you can be starving and still be fat. If you started at morbidly obese then you could have extreme anorexia but not "look anorexic" even after struggling for a long time

Eating too much likr bullimia is an eating disorder. Lot of people eat not even when they're hungry but just to cope with stress, sadness, joy etc.
But those obese women def have also a hormonal problems to trigger hunger like that
