That unusual shape on the pepper is just a natural deformity that happened while it was growing. Sometimes, during the early stages of fruit development, a small bud or extra bit of tissue starts forming on the surface. Instead of stopping, it keeps growing and ends up forming what looks like a little offshoot. This happens because of irregular cell division or a slight hormonal imbalance in the plant — things like auxins and gibberellins, which control growth, can behave unpredictably if the plant experiences stress, temperature changes, or even minor damage from insects.
