Face shape, palate expanders and double jaw surgery

I have a reallyyy bad asymmetrical underbite so I'm getting double jaw surgery hopefully soon but I feel like results from people with my severe of an underbite always leaves that long narrow face shape even when the bite is fixed. Should I consider using a palate expander now before the surgery or beg my surgeon to do a segmental lefort to expand the palate as well (unlikely as I'm getting the surgery free on the NHS). Would it be worth trying to go private with insurance just to get more discussion with what I want done? Or is there anything I can do post surgery to fix the narrowness? Would the only fix be like some crazy lefort 3 and should I just cope after the surgery? I'm not sure exactly what the issue I'm describing is, I have pictures on my profile from when I first joined if that helps.
