Will make this guide because a lot of people are misinformed about this theme, but first I want to say: if you have gyno, you’re cooked. Fitted shirts will look like shit on you, you will look weird shirtless, and it can fuck up your posture if you are very aware of it and trying to hide it all the time, arching your back and putting your shoulders forward. fortunately it can be fixed relatively easy (weight loss or surgery depending your case) first we have to identify between two types of gyno:

real gynecomastia: Real gynecomastia happens when, due to hormonal issues (either ongoing or during puberty), your mammary glandular tissue grows, so fat is not the problem. In this case, body weight is irrelevant, you can be at 9% body fat and still have enlarged glandular tissue, giving you those goofy aaah ugly titties. If you’ve had this issue for more than a year, you’re cooked, it’s no longer reversible, and surgery becomes the only real option. The surgery involves making a small incision at the bottom of the nipple and removing the glandular tissue. It’s not a major procedure, it’s ambulatory, and recovery usually takes around two weeks. If you’ve had it recently (usually less than a year), it can often be treated with pills like tamoxifen, which may reduce the glandular tissue and symptoms if used early.

pseudo gynecomastia: This one is tricky. Pseudo gynecomastia is an excess of fat in the nipple area that doesn't involve glandular growth. People assume this type of gyno only affects fat people, but that’s not true. Some people are genetically cursed and store a significant amount of fat under their nipples even if they’re extremely skinny, giving that puffy-nipple look. If you’re overweight, of course you’ll have big tits, lose weight and you’ll be fine. If you lose weight and still have big ass titties, then you probably have real gynecomastia. If you’re skinny with puffy nipples, you’re cooked. It doesn’t matter if you drop your body fat extremely low, you’ll still store fat in the nipple area because of genetics. In that case, the only solution is liposuction. Aqualyx can be considered but it's usually not as effective/aesthetic (In this case you won't get your glandular tissue removed).

How to diagnose it: To know exactly which type you have, the best exams are a mammography and hormone blood work, which will pretty much tell you what the problem is. You can also do a basic self-check by touching the affected area on your chest. If it feels soft, like regular fat, you probably have pseudo gynecomastia. If you feel something hard or rubbery under the skin, it’s probably real gynecomastia.
