I'm not an expert on surgery, but both you're lower jaw and chin are recessed, so I'm pretty sure you'd need both

Forehead Contouring to soften or smooth any protrusions or indentations. A Rhinoplasty. Cheekbone Augmentation (Malar Implants or Filler). Fat Transfer for undereyes or implant for support. Maybe a Genioplasty and Jawline Contouring.
