what should i expect? For anyone who has got or has had mse how bad is the pain? i am also getting braces on the upper row of teeth and im also gonna wear retainers on the lower row of teeth. Are there any aesthetic benefits from mse other than the health benefits?

I had MARPE (the one with miniscrews) and I still have it in. The pain is horrible at first, I know it sounds scary but I want to be honest. It sucks.

It's hard to judge how much it changed my face since I barely took any pictures before.

i talked with my orthodontist and she recommended i do RPE / hyrax which is basically a tooth anchored expander anchored to my molars. She said this is more suitable for me since im still 13 and my bone growth is still not done and i have open palates (i think thats what it was called). I saw a few posts saying that my teeth will tip but i dont think its possible for at least preteens to teens? The people who got their teeth tipped were adults already at there maximal bone growth peak. What do you think? When i was really little around 6 i had a removable expander and from what i remember it was kinda uncomfortable but not really painful and i had a tooth gap.
Honestly im not really doing it for cosmetic appearance but so that i can finally sleep, breathe and chew normally without the problem of my tongue not fitting and me 24/7 accidentally biting it BUT.. will it promote under eye support? And does under eye support help with canthal tilt?

What do you mean with teeth tipping?

It’s not going to help you with under eye support. Under eye support also can be linked with canthal tilt. Sometimes a lack of support under the eye area can cause a nct

teeth getting pushed outside from the alveolar bone from the force rather than promoting maxilla widening
Thats a genuine concern with toothborne expanders. Since ur 13 it might help, but ill be 100% honest with you, by age 13 your maxilla is mostly done growing. If you want real, real, aesthetic improvements (cheekbones, under eyes, midface, etc), you need to get alot of actual expansion with something that is directly anchored into the maxilla. Look through my postings, you'll see ive got some before and after aesthetic results in there and I explain the deal. I would reccommend asking your orthodontist to just go the skeletal route to ensure you get actual expansion. Larp about breathing issues as welll and say you can't breathe through your nose. One of the reasons for not being able to breathe through your nose is a narrow maxilla, since the nose hole opening is literally in the maxilla. Make sure they place it as far in the back of your mouth as possible (so farthest posterior placement) so that it maximizes the midface and cheekbone ehnacements and minimizes any form of nose widening. Couple this with alot of hard chewing and mewing for best results.

i also thought that maybe a tooth anchored expander is too late for me but im pretty sure my maxilla isnt developed yet but monday im gonna meet with a second orthodontist to hear his opinion. plus i dont know if in Bulgaria MARPE or SARPE is eligible for minors especially for under 14 y/o since its pretty invasive.

also at 13-14 y/o (as a girl) its your last year where your maxilla grows to its peak not when it stops im pretty sure but RPE has been successful for other girls my age range

i dont have MAJOR breathing problems tho and i dont see any point in getting that much of an invasive orthodontic surgery for little mm difference.

of course before i get the expander i will go under an x ray to see if the suture is closed or not but im pretty sure its flexible enough for the widening i need.

I forgot to mention another thing. expand super, super slow, about 1mm per month. That way you can also avoid getting a large gap in your teeth. These issues like the one you present come from when people do 'rapid' expansion, which is rather stupid, especially since your suture is still very soft, you don't need much force to 'crack' the suture open. They will not need to do a suture cut on you because of that, all they will do is screw into the maxilla. You should watch some videos of it being installed, it looks far closer to orthodontic treatment than surgical treatment. Its not as bad as you'd think. If I had known about this at age 13, I would have absolutely tried to get one. Now im 20, and its much, much harder to get.

While I completely understand if you don't want an invasive procedure at all, I will let you know that those few mm of bone are what determines your face. It's the few mm of projection here, and the few degrees of angle here, etc that count.

Idk how the healthcare system in Bulgaria works, but its worth an ask.

I think the toothborne expander you have may help, but if you really want to ascend, you need a bone borne thing.

Regardless don't feel forced to do anything you don't think is a good idea. I'm just letting you know the facts

ill definitely ask the ortho. Ill go under an xray so they can see if im suitable for the toothborne expander, but if my suture isnt closed do you think ill get the same expansion from a tooth anchored expander as sarpe or marpe?

No, you most certainly will not. Having the appliance anchored to the maxilla ensures that when the appliance expands, the maxilla moves with it. You can also put way more force on the maxilla, since the appliance is solid metal and directly anchored to the maxilla. I cannot overstate the difference in maxillary expansion with a marpe vs a toothborne expander. With the toothborne expander, you will maybe, at best get a few mm. With a bone anchored one, you get get close to 10. easily 4-5x the expansion.

SARPE (surgically assisted rapid palatal expansion) is not for you. You absolutely do not need any surgery to get palate expansion, you most likely do not even need any sort of piezo midline cut. Since you are a girl and 13, your suture is still more than soft and malleable enough to split without any type of surgical intervention. If someone offers that to you, thats a red flag. Tell them no, and that you would like to try with just the screws anchored to the maxilla and no cut. MARPE (miniscrew assisted rapid palatal expansion) is, in my opinion, the optimal choice for getting maxillary expansion (However, you should expand slowly, and not rapidly). Also, when doing maxillary expansion you need to watch carefully to make sure you aren't getting asymmetrical or getting uglier. Thats another reason why its best to go slowly, since you can track those things much better.

For your lower jaw, the mandible, however, you will need surgery to get any changes on that. Since you are still young and have some growth left, I wouldn't worry about lower jaw surgery at least for the next few years. Alot of development can happen in that time, even if you feel recessed now, don't worry yet. Spam the fuck out of hard chewing and mewing.
