I think it should mostly be covered by insurance personally but I also don't feel like an insurance's job should be to decide what you do or don't deserve treatment on anyways, and their primary job should be helping you pay for medical treatments (cosmetic or not) because you pay a monthly premium for that additional assistance.

But because health insurance is required in the US and is mostly privately owned (aka there's no alternative really) the main objective is to save and earn money, not actually be helpful or beneficial. Unfortunately it's the American way.
