Fix your lower third, yuck.

Also, your fWHR is cucked due to your lower third and long midface. You don’t actually need a Le Fort II jfl, it’s kind of overkill. You can just get bimax or a high cut Le Fort I with CCW + BSSO. But the reason your midface is so cucked is MAINLY due to your poor lower third and slightly long midface.

If you take a look at these images, it’s still a Le Fort I, but the cuts almost reach the lower orbital region. This can add some slight projection to your orbitals, similar to what a Le Fort II would do, and some people might argue it adds even more projection than a Le Fort II. However, it mostly depends on the type of Le Fort II cut you have.

Whereas a Le Fort II typically requires a coronal incision most of the time, where they essentially peel half your face off JFL.

For your case, something like this would probably turn out great. The example image here is a normal Le Fort I cut + BSSO and genioplasty. Since you didn’t provide any side profile pictures, I’m not sure whether your mandible is recessed or not, but what you need is to add length to the lower third, so you need something like mandibular downgrafting.

I honestly don’t think you need a genioplasty, because from what I see, your chin looks fine, and any added length would mess up your harmony. The downgrafting would already do its job, and also, with the addition of CCW to the mandible, it would project your chin even more.

You also need gonion implants, since you clearly lack them and there’s zero projection whatsoever.

Your lips are beyond fucked. I don’t even think a lip lift would save you, since you lack mass it would still look bad, just raised upwards. Your philtrum is horrible, you barely have any zygomatic projection, and your nose is terrible. I could keep going.
