you have to have functional issues with breathing for example and they can change some mandibular asymmetry with a bsso

theres not really a specified amount, but you should know whether surgery would be cosmetic or not due to problems you may or may not be dealing with because of your underbite. do you have issues with your teeth, difficulty chewing, sleep apnea etc.? if so then it could be argued a lefort 1 would not be cosmetic
