when people have depression or other mental illnesses it can give them a sense of numbness and derealisation
they may self harm due to attempting to feel something, same reasons alchoholics drink and addicts take drugs
it may also be a form of self punishment which then ramps up to suicide stemming from feelings of worthlessness and self hatred, they feel they deserve to be hurt and in pain unfortunately
very sad stuff
