Serious Class 2 Camouflage Regret: Planning a CCW Bimax or classic one after Premolar Extractions (Flat profile & Recessed chin)

Background:

I previously underwent a Class 2 orthodontic camouflage treatment. The orthodontist extracted two premolars to pull my upper front teeth back and close the gaps.

The Current Aesthetic Issue:

While my occlusion (bite) is now fully closed and functional, the camouflage treatment ruined my facial harmony.

  • Flat Upper Lip: Pulling the teeth back stripped away the skeletal support for my upper lip, leaving it looking flat and retracted.
  • Severe Mandibular Recession: My lower jaw is still severely recessed (Class 2 skeletal profile), my chin is weak/fuyant, and my neck line lacks definition.
  • Straight Incisors: My front incisors are currently very straight, not retroclined.
  • Nasolabial folds
  • Labiomental fold

The Dilemma & Surgical Strategy:

I want to go for a bimax. However, my local recommanded surgeon told me that a traditional orthodontic decompensation (reopening the premolar spaces) is highly risky for my roots and kinda complex.

Therefore, I am looking for a Bimax with Counter-Clockwise (CCW) Rotation using a "Light Coordination" or "Surgery First" protocol.

The goal is to preserve my current occlusion as much as possible while using the CCW rotation as leverage to:

  1. Advance the maxilla horizontally to restore my upper lip volume and projection.
  2. Advance the mandible massively in a rotational movement to maximize chin projection, shorten the lower third (face compaction), and sharpen my jawline—all without the need to reopen the extraction gaps.
