Introduction
A common notion I’ve seen on this forum and social media is that filler dissolving is seen as an undo button. Most people assume that if they don’t like their filler, dissolving is quick and simple.
Hyaluronidase is a useful medical tool for emergencies like vascular occlusion but elective dissolving isn’t predictable
1. Hyaluronidase doesn’t only target filler
Hyaluronidase is a protein enzyme which only targets hyaluronic acid, which is used in aesthetics to dissolve HA dermal fillers. HA fillers are made from cross-linked hyaluronic acid and the body also contains its own natural hyaluronic acid ,meaning the enzyme isn’t able to distinguish between the two.
A 2024 review discusses this mechanism and notes that native HA is generally replenished over time.

2. Not every filler dissolves the same
Different fillers behave differently varying on the brand, age, depth of the injection, concentration and degree of cross-linking
some dissolve quickly while others require multiple sessions

3. Migration is complicated
Filler doesn’t always stay exactly where it was injected. If the filler has migrated you need treatment in many areas. Also the removal won't be complete but ultrasound can help identify remaining filler

4. Results aren’t always immediate
Just because you inject enzymes doesn’t mean the filler disappears at the moment, in reality there will be swelling immediately after the treatment and reassessment after days/weeks. You can always get additional sessions if needed.

5. Risks
Now this doesn’t usually happen but there are potential complications like allergic reactions, swelling, bruising, temp volume loss, asymmetry and dissatisfaction AGAIN if too much filler is dissolved.

6. Emergency vs aesthetic dissolving
Hyaluronidase is used during emergency during vascular occlusion, eye threatening risks and severe overfilling. So in emergency uses hyaluronidase is sight and tissue saving.
For cosmetic uses, hyaluronidase dissolves old filler, changes shape or helps starting over.
