Usually, non-hormonal shrinkage of the penis is pretty rare and non permanent. While an increase in fat around the pubic region can give an impression of apparent shrinkage, forming gradual changes due to natural aging may have their say as well. Surgical removal or reduction options do exist but are usually employed in gender-affirming treatment as opposed to just purely cosmetic methods.
A better way to shrink your penis is by hormone therapy with estrogen and anti-androgens like cyproterone acetate, spironolactone, or GnRH agonists can cause hormonal shrinkage. Over time, this treatment reduces testosterone into the female range, which results in smaller testicles, fewer spontaneous erections, a softer, less veined appearance, and a decrease in the volume of erectile tissue. Usually partial, shrinkage affects both length and girth but never completely disappears. Because self-administering hormones can jeopardize bone density, heart health, and fertility, proper medical supervision is crucial. Unless treatment is discontinued early, changes that take place gradually over months or years are generally permanent.
