It's where a user (or users) intentionally increases their reputation points beyond what could be organically obtained. So, spam liking each other posts is technically a rep farm, counting this user wouldn't of obtained these hundreds or thousands of reputation points in any other context. The reason why this is difficult to address directly is because reputation points are treated differently here, especially when compared to other forums.
